According to Order No. 377, pay grades according to the unified technical system are established for this category of workers, taking into account the qualification category, within 5 years from the date of issuance of the order of the health care body (institution) on the assignment of the qualification category.

Three months before the expiration of the qualification category, the employee has the right to apply in writing to the certification commission to undergo recertification in the manner established by the Order of the Ministry of Health Russian Federation dated August 9, 2001 No. 314 “On the procedure for obtaining qualification categories” (hereinafter referred to as Regulation No. 314).

The certification commission is obliged to within three months from the date of receipt certification materials to consider them for assignment of a qualification category.

In the presence of good reason upon the proposal of the head of the institution, the period for recertification of a specialist may be postponed by three months. During this time, the employee is paid a salary based on his qualification category.

If a specialist refuses the next recertification, the previously assigned qualification category is lost from the moment the five-year period for its assignment expires.

In accordance with Regulation No. 314, when obtaining a qualification category, the professional qualifications, competence of the employee, as well as his ability to perform official duties in accordance with his position are assessed.

A specialist has the right to receive a qualification category in specialties that correspond to both the main and combined positions.

Specialists who have expressed a desire to receive a qualification category must submit the following documents to the certification commission: an application, a completed certification sheet and a report on work for the last three years - for specialists with higher vocational education and for the last year - for workers with secondary vocational education, approved the head of the organization in which the specialist works.

Application for confirmation of an existing qualification category or for obtaining more highly specialist has the right to submit to the certification commission three months before the expiration of the qualification category.

In accordance with clause 2.3. Regulation No. 314, a specialist has the right:

“- obtain a qualification category in the certification commission created in accordance with these regulations;

- get acquainted with the documents submitted to the certification commission;

- get clarification from the manager in writing, in case of refusal of the manager to sign the work report;

- undergo advanced training in the specialty;

- submit an application to the Central Attestation Commission or the body under which the attestation commission was created, in case of disagreement with the decision of the attestation commission.”

To exercise the rights of specialists to obtain a qualification category, the following certification commissions can be created:

· Central Attestation Commission - the Ministry of Health of the Russian Federation;

· certification commissions - by health authorities of the constituent entities of the Russian Federation, the Federal Department of Medical-Biological and Extreme Problems under the Ministry of Health of the Russian Federation, centers of state sanitary and epidemiological surveillance in the constituent entities of the Russian Federation;

· certification commissions in healthcare institutions, educational and scientific medical institutions - the Ministry of Health of the Russian Federation, other ministries and departments according to their subordination.

The structure and composition of the certification commission is determined and approved by the body under which it was created.

In its activities, the certification commission must be guided by the current legislation of the Russian Federation, regulatory legal acts of the Ministry of Health of the Russian Federation, Regulation No. 314, as well as orders of the body under which it was created.

Qualifications are assessed and recommendations are given for further vocational training specialist by voting in the presence of at least 2/3 of the number of members of the approved composition of the certification commission.     Preparation for assignment to the category of dentists

    Stage 1. Creation of a commission that makes a decision on assigning a qualification category to a doctor.

    In order for a dentist to confirm his knowledge to obtain a new category of dentist, he must undergo certification. The doctor is assessed by an attestation commission, the procedure for creating which is indicated in clause 12 of the Procedure approved by Order of the Ministry of Health of Russia dated April 23, 2013 N 240n. Within the commission there is a coordinating committee and groups of experts.

    For each specialty in which candidates will be certified, a separate expert group is assembled.

    According to clause 14 of the Procedure, the certification commission must include:

    • chief specialists of medical and pharmaceutical organizations;
    • specialists from non-profit professional medical organizations;
    • representatives of the government body or organization that assembles the commission;
    • representatives of the organization in which the candidate works;
    • other persons.

    Clause 14 of the Procedure also notes that a specific list of commission members must be approved by order of the government body or organization that forms this commission.

    Stage 2. Submitting a package of documents for consideration by the commission.

    Documents should be submitted to the organization or government agency, who convenes the commission, no later than four months before the end of the dentist’s category. It is possible to submit papers both in person and by mail. Clauses 20 and 21 of the Procedure list the list of required documents:

    1. Application addressed to the chairman of the commission signed by the doctor himself. It should contain the following data:

    • Full name of the applicant;
    • what category he would like to receive;
    • information about the current dentist category, including the date it was received;
    • consent to receive and process the doctor’s personal data;
    • date of document execution.

    2. Certification sheet of the dentist, the form of which can be seen in the first appendix to the Procedure. The printed sheet must be certified by a personnel specialist.

    3. Information about the work done for a certain period. For doctors with higher education it is three years, with an average of one year. The report consists of two main parts:

    • description of the work performed by the dentist;
    • summing up professional activities and developing options for its improvement.

    The document must be signed by the dentist himself, as well as his employer; The organization's seal is also required.

    If for some reason the manager does not agree on the report for assigning the category of dentist, the specialist may require an explanation of the reasons in writing. He includes the received paper in his package of documents.

    5. A certified copy of the work record and diploma of higher or secondary education special education, as well as other documents (certificates, certificates, etc.).

    7. If the doctor has changed his last name, first name or patronymic, a document confirming this fact is required.

    Clause 21 of the Procedure clarifies that if a dentist submits a package of documents late, an interview for assignment of a new category can be held after the expiration of the current one.

    Stage 3. Reception of documents by the commission.

    The procedure for accepting documents from doctors is also regulated by the Procedure:

    • when the documents are submitted by the dentist to the commission, they are entered into the registration journal on the same day;
    • then they check the correctness of filling out the application, compliance with the requirements for drawing up the certification sheet, as well as the presence of all necessary documents.

    If any errors are detected in the design or the package of documents is incomplete, the commission will refuse the doctor to accept the application. A letter of refusal stating the reasons must be sent within a week. Having received it, the dentist will be able to correct the mistakes made and again submit his documents for consideration by the commission.

    What is the procedure for certification of dentists for the category?

    Stage 1.Verification of professional skills.

    A dentist can receive a higher category only if he successfully passes an exam consisting of three parts (clause 7 of the Procedure):

    • evaluation by specialists of a report on the work done prepared by a doctor;
    • passing the test;
    • face-to-face interview.

    The purpose of these tests is to test the level of knowledge and skills of the dentist and make sure that they really correspond to a higher category. It is those skills that are directly related to the work in the specialty of a given doctor that are assessed.

    According to clause 18 of the Procedure, the commission has the right to evaluate work for the category of dentist only if at least half of all its members are present at the meeting.

    Clause 19 regulates the keeping of minutes of the meeting. The secretary fills out the protocol, and after the meeting it must also be signed by the chairman and other members of the commission. The form of this document is given in the second Appendix to the Procedure.

  1. Review by the commission of the received package of documents. According to clause 17 and clause 24 of the Procedure, 30 days are allotted for this.
  2. Also, the doctor’s work report must be reviewed within 30 days. Based on the results of its study, the commission issues an official conclusion.
  3. Within thirty days after submitting the documents, the date and location of the dentist examination must be determined. The specialist must find out where and when the exam will take place at least 30 days before the scheduled date. This information must be communicated to the doctor personally, and can also be additionally posted on the Internet at official page organizations and at information stands. Clause 16 of the Procedure allows for remote examinations, as well as the format of an on-site meeting of the certification commission.
  4. Interview and testing. Clause 24 of the Procedure establishes that the interview and testing must be carried out no later than 70 days after the dentist has submitted documents to the commission. As for testing, clause 25 of the Procedure establishes that a result is considered successful when the doctor correctly solves 70% of the test tasks.P. 26 of the Order states that proceeding to the interview is possible only when the dentist has successfully completed the test part of the exam. Experts must find out whether the candidate’s level of knowledge and training corresponds to the category of dentist for which he is applying. To do this, members of the commission will ask questions regarding theoretical and practical aspects work in the certified specialty.
  5. The decision to assign or refuse to assign a new category to a doctor, which is made based on the results of testing his knowledge (clauses 19, 27 of the Procedure).

All present members of the certification commission take part in the voting. To decide whether to assign or not assign to a dentist new category, a simple majority of votes is required. If the votes are equally divided, the decision is made by the chairman of the commission.

According to clause 19 of the Procedure, if the candidate is a member of the commission, then he cannot participate in voting on assigning a category to himself.

Clause 27 of the Procedure contains a list of reasons that allow members of the commission to decide to refuse to assign a category:

  • negative assessment of the report on the work done, which the dentist provided to the commission;
  • failure of the candidate to pass the test part of the exam (less than 70% of correct answers);
  • failure of the doctor to appear at the organization on the day of testing or interview.

According to paragraphs 28, 29 of the Procedure, the decision of the commission (if it was decided not to assign a category, the refusal must be justified) is recorded in the minutes of the meeting and in the dentist’s certification sheet.

Clause 19 of the Procedure provides that a member of the commission has the right to disagree with the final decision. In this case, he can express his opinion in writing and attach the paper to the protocol.

Stage 2. Issuing an order to assign new categories to dentists and transferring the relevant documents to them.

Clause 32 of the Procedure provides that, based on the results of meetings of certification commissions, an administrative act of a state body or organization is issued on the assignment of qualification categories to doctors.

Clauses 33 and 34 regulate the procedure for notifying the dentist about the decision taken. To do this, the secretary of the certification commission is obliged to:

  • make an extract from the relevant order, which reflects the results of certification and the assignment of categories of dentists to specialists;
  • deliver the extract to each dentist personally, or arrange postal delivery. The deadline for delivery of the extract is also established by law - no later than 120 days after the doctor submitted the documents for registration;
  • enter information about the delivery or mailing of the extract into the document registration journal.

Stage 3. The doctor appeals the decision taken by the commission.

If a dentist believes that his certification work for the category of dentist has been assessed unfairly, he can file a complaint with the government body or organization under which it was formed. Clauses 16, 35 of the Procedure establish that the doctor has the right to challenge the decision within a year after the decision is made.

Among working professions there are many categories - from the first to the sixth. For dentists, everything is much narrower. There are second, first and highest categories of dentist. Receiving them is regulated the federal law, as well as orders of the Ministry of Health.

Basic category of dentist

First, doctors are awarded the second category - this is the basis, then the first, and the most experienced and skilled receive the highest.

To receive his category, a dentist must meet certain requirements.   • a printed qualification sheet, which is certified by the human resources department of the medical institution where the specialist works;
  • certification report on professional activities, which must be agreed upon with the specialist’s supervisor and certified with a seal. This report includes an analysis of the doctor's professional work activity over the past 3 years.

The report should look like this:

  • The first chapter includes information about the medical institution where the dentist works, about the dental department of this institution, the technical equipment of the dental office and, directly, the doctor’s workplace.
  • The second chapter contains a report on the dentist’s activities for three years. Here it should be analyzed what the dynamics of the work are, whether it is positive. It should be covered how a dentist masters modern technologies and treatment methods. It also provides statistics and qualitative and quantitative performance indicators in the form of tables or graphs. Brief conclusions (1-2 sentences) should be drawn for tables and graphs. It would be welcome if it is mentioned what treatment methods the specialist uses in his work.
  • The third chapter contains an analysis of new methods of prevention and treatment.

A doctor’s certification report on the work done is a scientific and practical work in which the doctor analyzes the results of his professional practice and career activities over the past three years on all issues of his specialty.

1. Preparation of the certification report

1.1. General requirements (in accordance with GOST 7.32–91 and international standard ISO 5966–82): The attestation report should reflect as much as possible the author's own contribution to the activities of the medical organization. The report must be printed on one side standard sheet white A4 paper, Times New Roman font, font size 12, line spacing - 1.5. 1.5.1. Design of tables. The table is indicated by the word “Table” and a number written in Arabic numerals in the upper right corner (the “No” sign is not indicated). This should be followed by a centered table title. Tables, depending on their size, are placed after the text in which they are mentioned, or on the next page. The link to the table in the text is formatted as follows: see table. 1. If there is only one table in the work, then the word “table” is not abbreviated: see table. Usually the first reference uses the word “see.” it is not written: “From the table. 1 it is clear that...” For further references, mark in brackets: (see Table 1). When using tables, keep in mind the following recommendations: If possible, you should not use the “sequential number” column (“No.”), since in most cases it is not needed. 1.5.2. Design of drawings and photographs: The name is written under the picture, preceded by the abbreviation “fig.” and the serial number is a number written in Arabic numerals (the “No” sign is not indicated). All this designation is centered below the picture. Photographs of macro- or microscopic specimens, prints of X-rays and ultrasound images, copies of ECGs and other materials must be of sufficiently high quality.

1.5.3. Application design: Applications, unlike other types of additional information, are located outside the text of the certification report. Applications may include text, tables, pictures, photographs, and drawings. All types of additional information in appendices are numbered in the same way as in the main part of the work. Each application must start on a new page. Applications are identified by the word “APP” in capital letters and a serial number (Arabic numerals) in the upper right corner (without the “No” sign). This is followed by a centered application title.

2.3. Analysis of the author’s own activities over three last year . The key point of the certification work. In this section, the author must isolate his personal contribution from the overall work of the department. The author must specifically indicate the amount of work performed, detailing the type of research over the last three years, and list in as much detail as possible what specific research methods he or she has. The author must back up his skills and abilities with a specific volume of research on various organs and systems. It is necessary to indicate which basic techniques you do not know, but would like to master in the near future and how.

2.4. Analysis of verification of own research: a comparison of diagnoses and conclusions over the last three years is carried out with data from a pathological examination of biopsy, surgical material or autopsy, an analysis of identified inconsistencies and discrepancies, and the reasons for their occurrence. If pathomorphological verification is impossible, clinical data over time and data from other research methods should be provided.

2.5. The most complex, rare clinical examples and observations: It is necessary to cite 2-3 of the most significant clinical observations in which the author, as a specialist, played a key role in making the correct final clinical diagnosis. Each clinical example must be supported by short form clinical, laboratory data, results of pathomorphological and other research methods. Prints of objective instrumental research methods (x-rays, ultrasound, ECG and other materials, macro-micrographs) must be of sufficiently high quality. Clinical examples should not be cluttered with imprints of all types of studies.

2.6. Abstract part of the work. The volume of this section should not exceed 5 pages of typewritten text. The desired context for this section is the formulation of a specific problem. For example: Possibilities of X-ray computed tomography in the differential diagnosis of laryngopharynx diseases modern stage: accumulated experience, problems and development prospects. In this section, we can limit ourselves to some purely narrow issues of diagnosing any specific disease, which have not yet been properly resolved at the present stage. In this section you can cite one of the published works of the author himself, including co-authorship. The abstract part of the work should not be devoted to a description of any known, generally accepted research methods or a description of the technical parameters of any medical device.

2.7. Conclusion. In a brief free form, the author discusses the practical activities of the department, his own personal contribution to the work of the department, the reasons that led to diagnostic defects and measures to eliminate them in the future.

2.8. conclusions. Conclusions should follow logically from the material presented. The clear formulation and clear context of the conclusions indicate the analytical abilities of the author of the work.

2.9. Practical recommendations and suggestions. The author's recommendations and proposals will be analyzed by the main freelance specialists of the Ministry of Health of the Republic of Sakha (Yakutia). Most relevant practical recommendations and suggestions will be taken into account for further implementation at a higher level.

2.10. Bibliographic index. The bibliography should not exceed 15-20 sources. It is advisable to indicate the most significant sources of the last 5 years, including original articles used in writing the report.

2.11. List of published works. If the author has publications, it is necessary to attach a list of his own scientific works indicating the output data (the title of the abstract of the report or article, the name of the collection or medical scientific journal, year of issue, place of publication, page numbers, etc.), titles of reports that the author presented at symposia, meetings of scientific societies and conferences over the past 3 years. If there are patents, inventions or innovation proposals, copies of them must be submitted.
